Cerebrovascular and endovascular neurosurgeon Risheng Xu discusses the management of chronic subdural hematomas, a common affliction of older adults. The management of these patients has changed over the past few years with the advent of middle meningeal artery embolization, a minimally invasive procedure which has been shown to significantly reduce the chances of subdural expansion. Surgical management of subdural hematoma includes bur hole evacuation, craniotomy and large craniectomy. Middle meningeal artery embolization can serve not only as a standalone procedure to manage chronic subdural hematoma; it can also serve as an adjunct procedure after surgical management to further decrease the chances of subdural hematoma re-accumulation. This is an exciting area with many new clinical trials on the horizon.
