Known for advocating evidence-based, preventative, and regenerative approaches, Dr.Mahadaviniya believes the future of aesthetics lies not in dramatic transformation, but in subtle, longterm rejuvenation that respects individuality.Here, he shares his insights on longevity, thread lifting, and why prevention is becoming the new gold standard in skin health.

How has teaching physicians around the world shaped the way you view your own role in the evolution of aesthetic dermatology?

Teaching globally has expanded my perspective far beyond my own practice. When you train physicians from different regions, you see how culture, anatomy, and patient expectations evolve, and you learn as much as you teach. It has taught me that my role is not only to refine techniques, but to help guide the field toward safer and more ethical outcomes. It keeps me accountable to evidence-based practice and to continuous innovation.

How have different cultures, beauty ideals, and patient expectations influenced your approach to dermatology?

Working in culturally rich environments has taught me that beauty is both universal and deeply personal. In Iran, patients often favour very refined, delicate enhancements; in the UAE, there is more openness to sculpted contours and visible rejuvenation. Ultimately, good dermatology is about respecting individual identity, not creating the same look for everyone.

In such a fast-moving field, what keeps you curious, and what continues to motivate you to refine and evolve your techniques?

Curiosity comes naturally when you treat the face as both an anatomical structure and a living, emotional canvas. Every patient presents a new challenge. What motivates me is the responsibility to offer treatments that are safer, more natural, and more regenerative. The industry changes quickly, but the goal never changes: deliver outcomes that stand the test of time. You emphasise that thread lifting is not a replacement for surgical facelifts, but a powerful preventative and rejuvenating tool.

Where do you believe thread lifting sits today within the spectrum of dermatological and aesthetic treatments?

Thread lifting is the bridge between injectables and surgery. Today, threads are best used for early ptosis, contour enhancement, and collagen stimulation. They are part of a long-term plan, not a stand-alone solution. You often highlight prevention as one of the most important benefits of thread lifting.

How is preventative dermatology changing the way we approach aging and skin health?

Preventative dermatology shifts the focus from repairing damage to slowing the ageing process itself. By improving collagen, suspending early sagging, and supporting tissue integrity, we reduce the need for aggressive interventions later. Patients now understand that ageing is cumulative, and prevention is more cost-effective, less invasive, and produces more natural results. We are seeing dermatology shift from surface-level treatments to deeper regenerative strategies.

What developments most excite you for the future of skin longevity and non-surgical rejuvenation?

I’m most excited about bio-stimulatory treatments, injectables, threads, energy-based devices, and regenerative cell therapies. These treatments work with the body rather than against it, helping tissue behave more “youthfully”. The future is personalised and regenerative.

What drew you to specialise in dermatology in the first place?

Dermatology allows you to combine artistry with science. It’s a field where even small changes can dramatically impact confidence. I was drawn to the precision, the creativity, and the ability to meaningfully improve quality of life.

What has been one of the most rewarding treatments in your career?

The most rewarding cases are never the most dramatic. They’re the ones where a patient looks in the mirror and recognises themselves again. Subtle rejuvenation, especially around the midface or jawline, can truly restore someone’s sense of identity.

What treatments do you think are over-hyped and which treatments deliver the best value for patients?

Any treatment that promises “instant, permanent results” is usually over-hyped. Overly aggressive fat-dissolving treatments, unproven collagen supplements, and many viral social-media “miracles” often don’t deliver meaningful long-term outcomes.
